Young people's 14 plus team

The 14+ / 18+ Team provides support to young people from the age of 14 up to the age of 25 years.

Young people eligible for 14+ Team support are:

  • 14+ LAC - The team manage and co-ordinate the support and care provided to all 14+ Looked After Young People from Northumberland. This is in line with LAC (Looked After Children) procedures overseen by an Independent Reviewing Officer (IRO).  
  • 16+ (Post year 11) CIN - The team provide Child in Need support to 16+ young people who require additional support where there are safeguarding concerns. This includes managing and coordinating support to address concerns such as homelessness, substance use, sexual exploitation, criminal exploitation, mental health.
  • Leaving Care support - Personal advisors provide leaving care support through pathway planning for young people who have left care. This includes 16 and 17 year old Relevant Care Leavers and post 18 Former Relevant Care Leavers. Support is provided in line with the Children (Leaving Care) Act 2000 and the Children and Social Work Act 2017.  
  • Transitions - The team support young people approaching adulthood and as adults, therefore there are transition pathways across all agencies, this includes children’s social care to adult social care / safeguarding, children's mental health services to adult mental health services, youth offending to Probation. A high number of young people in our team require this support. Social Workers in the team coordinate the plans and supports  for young people transitioning to adult services.
The 14+ Team being based within Northumberland Adolescent Service (NAS) promotes positive multi agency working and ensures young people are able to access the appropriate support as they move into adulthood.
