How can I get support from Shared Lives?
You will need an assessment to see
whether
you are eligible for support.
To request an assessment you can contact the team at
SS-SharedLives@northumberland.gov.uk
If you have already had an
assessment
please contact your social worker
or care manager
to find out more about support.
