
Northumberland Care Trust contracts with a
number of Providers of Residential, Nursing and Home Care.
The overarching principle is that all services
purchased must have a Contract in place and on the client file and
must be recorded in SWIFT. To assist with this, the contracts
are accessible below and include the correct rates. For each
contract there is a package available in SWIFT which matches the
rate on the Contract.
Where Care Managers send out the Contract, the care package
should be linked by District admin staff. If for some reason
you can’t find a package in SWIFT which matches the rate on the
Contract, please contact Contracts on 01670 622437 who will be able
to help.
If there is no Contract available for the Provider which you
require, please complete a contract request form by following the
links
NTW and Head Injuries staff will only have access to this folder
if they are signing on at an office which has the NHA network and
they have the relevant NHA logins.

which is issued by the Care Manager, form the Contract between the
Care Trust and the Provider.

Statement of Terms
Monitoring process
All older people care homes within Northumberland undergo a
review process each year. The process is called the Quality
Weighted Payment Scheme (QWPS), and as result homes are placed
within a banding structure. Band 1 being the highest result
possible and Band 4 being the lowest result.
Below is a table showing the bandings for each of the care homes
in Northumberland following the QWPS review
